pikir- pikir program last2 minit sempat juga keluar tgk harry potter, last minit pgram lac tuh, dah la last minit, jalan jemmed, kereta byk btul, biasa lah wktu org kluaq pejabat, nsib baek sampai2 sempat beli tckt, azam, luqman, iqbal, farazi, pait janggut, 6 org kami jer lah, saja kuaq cuci mata, dah bersawang dah otak dduk di rumah lama sgt, maklumlah , malas nk kluaq bila dah start cuti nie, movie start jam, 6.15, nasib baik la pakai jam 15 minit awal, punya la rushing, mcm nk ligan naga, sampai2 tiba2 ticket harry potter jadi haga RM 8, mmg la panas, nasib baik la cukup duit, maklum lah takut terlepas cerita, duit dalam wallet pon x check tinggal berapa, kalut masuk dalam panggung, panggung problem air cond pulak, memang sakit hati, harga mahal, tp sevice teruk, dah la tuh dalam 20 minit delay, baru nak mula, kalau taw tdi, buat lambat2 pon x pa. Harry Potter, semua orang kenal, dah besaq dah si Daniel Radcliffe tuh, anak teruna la katakan, baya2 aku ja... tp storyboard tuh slow sgt, bosan tgk, x dak la aksi2 bahaya tuh, dah macam filem bercakap ja, sampai mengantuk-ngntuk dalam panggung. tp totally boleh diberi thump up, ok la... tp mmg x sabaq nk tgk final of the harry potter.. yg jadu fenomena pada semua tuh

dah seminggu arwah meninggalkn kita, mmg terasa akan pemergian arwah, namun arwah pergi dgn segala persiapan rapi bagi untuk kami menyambung sisa-sisa perjuangan arwah, byk membawa kesan kepada kami, arwah masih igtkan aku semasa sebelum beliau menghembuskan nafas nya yg terakhir, benar kata org tua2, harimau mati meninggalkan belang, manusia mati meninggalkan nama, byk pessan dan tanggungjawab yg perlu digalas atas bahu ini, berhadapan dgn situasi mencabar begini, memang memerlukan kesabaran, insyallah Alllah akan kurniakn yg terbaek...Pesan arwah, jdi manusia terbaek, jaga diri, terus belajar, dan jgn mengalah, satu pesan arwah yg x dpt ditunaikan, ialah x sempat nk menguruskan jenazah arwah, arwah berpesan, supaya jenazahnya diuruskan oleh budak2 pengakap, namun ribuan halangan melanda, insyallah satu lagi hajat arwah, akan aku laksanakan, tp arwah menutup mata sebelum ianya sempat di laksanakan, bersanding dgn uniform pengakap, akan kubuat, hajat arwah nk melihat tidak kesampaian, namun sebagai menyanjung perjuangan arwah, Insyallah diberi kekuatan melakukannya

Kehilangan beliau amat dirasai oleh setiap anak2 pengakap juga penduduk2 di Yan. beliau yg selalu berbakti kini tiada lagi di dunia. Mana hendak ku cari yg seperti beliau, yg selalu mendendangkan halwa telinga, menceriakan yg suram, memberi semangat motivasi lantas membangun utuh jiwa muda mudi untuk berbakti. tiada lagi kata nasihat pd yg dahaga akan nasihat beliau, yg mencari ilham lagu beliau, yg setia menyokong penuh akan perjuangan membela bangsa dan negara. alhamdunillah.... puasa sudah... Syawal pun menjelma, tiada yg paling gembira melainkan berkumpul di samping sedara, mara... kwn2,.. dan semua.... nikmat di hari raya dapat menjamah ketupat dan seynding pada pagi raya, dpt balek dan cium tangan kedua org tua, dpt berkat dari itu, dan masih lgi mendapat duit raya. penat dgn rya pertama di rumah tok, rya kedua di rumah opah, ketiga, keempat hingga ke sepualuh di rumah kwn2.... menjadi tradiri Alpha Jerai Rover , ada kan jamuan raya mengikut urutan, jatuh di rumah sndiri, tp sebelum tuh ke rumah saudara Syiken di alor setar rombongan beramai2 bersama yg lain, hari seterusnya di rumah sendri dan rumah saudara Husna. penat dgn raya, berat pun dah naik 7 kilo, mudah kata 10 hari cuti . misi selepas ini turun kan semula lemak raya, tp thankz to mama, tlg managementkan open house Alpha Jerai Rover dgn menu yg padat dan byk, daging masak hitam, gulai rebung dan udang, burung puyuh dan ayam masak merah, sayur dan nasi minyak.. thankz pd abah support majlis tersebut. bukan senang cari rebung.. tp berbaloi bila dah dpt... apa pun selamat hari raya mohon maaf zahir dan batin.

Alhamdunillah... seelah masok hari ker 13 berpuasa.... ada mendapat undangan ke majlis berbuka puasa di Maksak Pahang... bersama Kesatuan Rugby Pahang... walaupun pasukan Peramah tidak mendapat johat dalam Liga Rugby Pahang Yg lepas, tp sekuarang2 dijemput ke majlis itu pun merupakan satu penghormatan bagi kami... Insyallah ada peluang pada next musim akan dapat terus berjuang memajukan sukan demi rakan2.. menjaga kseihatan dan demi minat rakan2 yg mendalam tentang sukan rugby ni... seterusnya ker majlis berbuka puasa di Mabiq di Tanjung Lumpur bersama anak2 yatim.. dpt bersama berkongsi nikmat ramadhan yg dikurnia merupakan satu anugerah buat rakan2 bersama membantu dalam mencari jariah akhirat... dgn makanan yg disediakan oleh wak Salleh... 9w2WSL.... dapat rakan2 berbuka puasa.. sedap.. namun kenyang bila dah makan sampai kepak di majlis di maksak tadi.. hanya mampu menjamah kueh2 saja.... terima kasih Peramah.......

Alhamdunillah.. hari nie genap seminggu berpuasa.... mcm2 ragam seminggu berpuasa.... dugaan makanan di bazar... dan yg x tahan dugaan YL YL di bzar.. yg tuh mmg berpeluh tahan dugaan mcm tuh di bulan puasa macam nie... dgn berebut nk bli juadah lgi.. menahan dugaan mata.... dugaan makanan lagi... maklum lah di bulan puasa nie... mcm 2 teringin nak makan... tp satu lagi masih dalam impian.. dah seminggu cari pulut sekaya di kuatan nie.. tp masih x jumpa.... dah 4 bazar ramadhan di rondai.... tp tetap x jupa jgk... mungkin masih x dak rezeki kot... Ada satu fenomena yg luar biasa di bulan ramadhan ni bila di kuantan..dqah macam satu pesta bila di tengok... mmg seronok... ramai2 berbuka puasa di padang MPK, sebelah dgn masjid... sgt meriah jika digambarkan... ada dgn bunyi mercun.. bunga api... tp suasana di sini jika hendak melintas ke masjid x ubah mcm medan perang.... dgn kena elak kn diri dari percikan mercun.. mmg dija dibayang kan agk sedikit mencabar... namun... semua itu akan hampa igt smpai bila2.......

got a splendid Saturday on last week, actually just have a task to teach some of the troop about the basic knot skill, 5 basic knot then the camp craft in scouting at Tanjong Lumpur School. got a really enjoy time with the cub scout... having fun scout song with them, then i get the passion back. something that i lost about a week. then it's not stop at there, still with the uniform, going to Pekan, got a launch with the other scout. the new kids that have a basic camp. then move on to the Jerantut, having the JOTA, Jamboree on the Air 2010. lucky on that day, i got transmitted to the Bintulu, then got someone from Hawaii.... having a dinner with all the scout...still with the uniform, i'm wearing the uniform fpr the whole day. then i get the passion back in my life....... i really love scout...

menjelang ramadhan, rezeki dan nikmat makannan mencurah2, penuh perut dapat diisi setelah dibiar kosong beberapa jam. Alhamdunillah, majlis berbuka puasa di adakan di Vistana hotel, thankz to Miss Dilla, and all the masscom student, Arif Sulaimini, SR student.... buffet berbuka puasa yg agak mewah, telah berjaya dikosongkan oleh student2 masscom yg sememangya lapar... tiada lgi cover ayu dan mancho bila lapar... semua bagaikan mengamuk, tak cukup dgn itu, di adakan sesi fotography id sekeliling ballroom hotel, dan juga di lobi. bagaikan lumrah sesigilalensa.... all the best.. cukup menarik event ini .... sekurang-kurangnya terubat jua hati yg rindu berbuka di kampung halaman.

alhamdunillah.... the last shoot with the child been finished up.. got to spent time.. the energy... the miney and everything for every single scene dat been done. 1 week shooting... then the story board been finish.. sometime it be hard to us when all the planning be got in damn. the printer, the CC pc put me in problem... make all the work became hard... but when all of us became one.. all be done in best... i'm be proud be one of the ISS advertising production.... but be ready with the next shooting on the Ramadhan.....

adakalanya manusia membuat silap, silap yg diri sendiri x akan nampak, tp bila kita buat silap tersebut kpd org yg dekat dgn kita, mungkin kita akan rasa serba salah, namun atas dasar ego, kita biarkan rasa seba salah itu berlalu saja, sampai masa kita akan rasa terlalu kosong atas kesalahan tadi,jadi kita akan kongsi kan perasaan tersebut dgn sesorang yg akan sedarkan kita dgn kesalahan yg kita dah buat. Mungkit kita pentingkan diri sendiri dan ego, lalu kita buang sedikit perasaan bersalah itu jauh dari sudut hati, lantas membuatkan kita menjadi semakin riak dan takbur, takbur atas kurniaan Tuhan yg diberi pada kita sekadar hendah menguji siapa diri kita yg sebenar. dan juga kita menolak semua orang atas sebab kita merakan kita lah yg terbaik, tapi bila sampai masa, semua sudah lupakan kita, kita tergapai-gapai sendirian, perasaan korong dalam jiwa itu lantas mengigatkan kita atas kesilapan yg dilakukan... perassan kosong itu menjadi kunci untuk kita sedar akan kesilapan lalu .. Alhamdunillah.. duit MARA dah msok.. dah bleh gunting rambut.. genap pkul 5.30 petang... sehelai demi sehelai rambut gugur.... gugur si kedai auntie depan rumah 8far dan Galah... atas sokongan rakan2.. jdi terpaksa gagahkan diri untuk potong rambut.. Alhamdunillah... nmpak imej sdikit 'clean' bila dah gunting rambut.... rambut pn dah jdi pendek.... rambut dah pendek.. jdi stick dgn jadual shooting untuk PSA Warisan.. yg akan memegang watak Bapa ..jdi sesuai dgn gaya rambut yg agak pendek dan kemas. Namun tugasan shooting untuk PSA tetap diteruskan dgn mencari rumah kayu antik sebagai salah satu element yg digunakan dalam PSA tersebut... Puas mencari di Kg. Tiram ..akhirnya teringat akan kediaman 9w2TMN.. di Bukit Ubi.. hajat dihati hendak minta izin beliau untuk melakukan shooting untuk scene di rumah kampung tp kelihatan beliau tiada.. so esok cari jalan untuk shooting di rumah beliau.....namun penat terubat bila dapat makan murtabak Mangkasar di bukit sekilau sambil usha YL uitm kt situ.... sedikit insiden berlaku 8far gopoh dan terlanggar ..seb baek LC baru beliau x calar truk.. huhu

KL hamfest festival.

Alhamdunillah duit MAra dah pun masok, tp terkilan sebab assingment yg byk jadi tak dapat ikut rakan2 membina repeater di Bukit Fraser, Hamfest pn hanya tggu si Iqbal, 9W2MRQ, balek dan bercerita pasal hamfest tuh.. huhuh nasib badan lah.. tp dapat jugak hari nie menghilang kn keboringan dgn ikut ke PIKOM PC fair, di megamall Kuantan.... jalan2 cuci mata jer.....

Overview

An arteriovenous malformation (AVM) is an abnormal tangle of blood vessels in the brain or spine. Some AVMs have no specific symptoms and little or no risk to a one’s life or health, while others cause severe and devastating effects when they bleed. Treatment options range from conservative watching to aggressive surgery, depending on the type, symptoms, and location of the AVM.

What is an arteriovenous malformation (AVM)?

Normally blood flows from the heart through large arteries to all areas of the body. The arteries branch and get smaller until they become a capillary, which is just a single cell thick. The capillary bed is where the blood exchanges oxygen and nutrients with the body tissues and picks up waste. The blood travels from the capillary bed back to the heart through veins. In an AVM, arteries connect directly to veins without a capillary bed in between (Fig. 1). This creates a problem called a high-pressure shunt or fistula. Veins are not able to handle the pressure of the blood coming directly from the arteries. The veins stretch and enlarge as they try to accept the extra blood. The weakened blood vessels can rupture and bleed and are also more likely to develop aneurysms. The surrounding normal tissues may be damaged as the AVM “steals” blood from those areas. There are four types of AVMs:

Arteriovenous malformation – abnormal tangle of blood vessels where arteries shunt directly into veins with no intervening capillary bed; high pressure.
Cavernoma – abnormal cluster of enlarged capillaries with no significant feeding arteries or veins; low pressure.
Venous malformation – abnormal cluster of enlarged veins resembling the spokes of a wheel with no feeding arteries; low pressure, rarely bleed and usually not treated.
Capillary telangiectasia – abnormal capillaries with enlarged areas (similar to cavernoma); very low pressure.

AVMs can form anywhere in the body and cause symptoms based on their location. Brain AVMs can occur on the surface (also called cortical), deep (in the thalamus, basal ganglia, or brainstem), and within the dura (the tough protective covering of the brain). Dural AVMs are more accurately called arteriovenous fistulas (AVF). The veins of the brain drain into venous sinuses, blood filled areas located in the dura mater, before leaving the skull and traveling to the heart. In an AVF there is a direct connection between one or more arteries and veins or sinuses. Dural AV fistulas and carotid-cavernous fistulas (CCF) are the most common AVFs.

Spinal AVMs can occur on the surface (extramedullary) or within the spinal cord (intramedullary) and are classified into 4 groups:

Type 1 - (most common) is a dural arteriovenous fistula. They usually have a single arterial feeder and are thought to cause symptoms by venous hypertension.
Type 2 – (also called glomus) is intramedullary and consists of a tightly compacted nidus over a short segment of the spinal cord.
Type 3 – (also called juvenile) is an extensive AVM with abnormal vessels both intramedullary and extramedullary.
Type 4 - are intradural extramedullary arteriovenous fistulas on the surface of the cord

What are the symptoms?

The symptoms of AVMs vary depending on their type and location. While migraine-like headaches and seizures are general symptoms, most AVMs do not show symptoms (asymptomatic) until a bleed occurs. Common signs of brain AVMs are:

Sudden onset of a severe headache, vomiting, stiff neck (described as "worst headache of my life")
Seizures
Migraine-like headaches
Bruit: an abnormal swishing or ringing sound in the ear caused by blood pulsing through the AVM

Common signs of spinal AVMs are:

Sudden, severe back pain
Weakness in the legs or arms
Paralysis

AVMs damage the brain or spinal cord in three basic ways:

AVMs can rupture and bleed into the brain—called an intracerebral hemorrhage (ICH), or it can bleed into the space between the brain and skull—called a subarachnoid hemorrhage (SAH). Small AVMs (less than 3 cm) are more likely to rupture than large ones. The bleeding can cause a stroke.
AVMs can grow large and create pressure against the surrounding brain, resulting in seizures and hydrocephalus. This is more common in large AVMs.
AVMs can reduce the amount of oxygen delivered to nearby tissues. Because the blood flows directly from the artery to the vein, cells that normally get oxygen from the capillaries begin to deteriorate.

The risk of AVM bleeding is 2 to 3% per year. Death from the first hemorrhage is between 10 to 30%. Once a hemorrhage has occurred, the AVM is 9 times more likely to bleed again during the first year. Patients often want to know their lifetime risk of bleeding if they’re weighing the risks and benefits of having surgery. Using the following formula one can calculate their risk (1):

Lifetime risk (%) = 105 - patient’s age

For example, a 25-year-old man has an 80% lifetime risk of bleeding (at least once). Many factors affect this percentage, including where the AVM is located and what type of AVM it is. It’s best to talk to your doctor about your own individual risk.

Who is affected?

AVMs of the brain and spine are congenital (present at birth) and relatively rare. Approximately 300,000 (.14%) Americans have AVMs, but only 12% of those experience symptoms. They affect both men and women at about the same rate. They can occur at all ages, but most often cause symptoms between 20 and 40 years of age. AVMs account for about 2% of all hemorrhagic strokes each year.

How is a diagnosis made?

Whether you or a loved one was brought to the emergency room with a ruptured AVM or are considering treatment options for an unruptured AVM, the doctors will learn as much about your symptoms, current and previous medical problems, current medications, family history, and perform a physical exam. Diagnostic tests are used to help determine the AVM’s location, size, type, and involvement with other structures.

Computerized Tomography (CT ) scan is a noninvasive X-ray to view the anatomical structures within the brain to detect blood in or around the brain. A newer technology called CT angiography involves the injection of contrast into the blood stream to view the arteries of the brain. This type of test provides the best pictures of blood vessels through angiography and soft tissues through CT.
Magnetic Resonance Imaging (MRI)scan is a noninvasive test, which uses a magnetic field and radio-frequency waves to give a detailed view of the soft tissues of your brain. An MRA (Magnetic Resonance Angiogram) is the same non-invasive study, except it is also an angiogram, which means it also examines the blood vessels, as well as the structures of the brain.
Angiogram is an invasive procedure, where a catheter is inserted into an artery and passed through the blood vessels to the brain. Once the catheter is in place, a contrast dye is injected into the bloodstream and the X-ray images are taken (Fig. 2).

What treatments are available?

Surgery, endovascular therapy, and radiosurgery can be used alone or in combination to treat an AVM. Endovascular embolization is often performed before surgery to reduce the AVM size and risk of operative bleeding. Radiosurgery or embolization may be used after surgery to treat any remaining portions of the AVM. Your neurosurgeon will discuss with you all the options and recommend a treatment that is best for your individual case.

Observation
If there have been no previous hemorrhages, the doctor may decide to observe the patient, which may include using anti-convulsants to prevent seizures and medication to lower blood pressure.

Radiosurgery
Radiosurgery aims a precisely focused beam of radiation at the abnormal vessels. The procedure takes several hours of preparation and one hour to deliver the radiation. The patient goes home the same day. After six months to two years, the vessels gradually close off and are replaced by scar tissue. The advantage of this treatment is no incision and the procedure is painless. The disadvantages are that it works best with smaller AVMs and may take a long time to show effect (during which time risk of hemorrhage exists).

In a recent long-term study done at Mayo Clinic, 73% of patients had excellent or good outcomes after radiosurgery and were protected from the risk of future bleeding (2).

Endovascular Therapy
Endovascular treatment uses small catheters inserted into your blood vessels to deliver glue or other obstructive materials into the AVM so that blood no longer flows through the malformation (Fig. 3). It is performed in the angiography suites of the Radiology Department by a Neuro Interventionalist. The procedure is performed under general anesthesia. A small incision is made in the groin and a catheter is inserted into an artery then passed through the blood vessels to the feeding arteries of the AVM. Occluding material, either coil or acrylic glue, is passed through the catheter into the AVM. The procedure time can vary, and the patient remains in the hospital several days for observation. The advantage of this treatment is it’s less invasive than surgery and can be used to treat deep or inoperable AVMs. Disadvantages include risk of embolic stroke from the catheter and rebleeding since the AVM is not completely obliterated. Multiple treatments may be necessary.

Surgery
Using general anesthesia, a surgical opening is made in the skull, called acraniotomy. The brain is gently retracted so that the AVM may be located. Using a variety of techniques such as laser and electrocautery, the AVM is shrunken and dissected from normal brain tissue. The length of stay in the hospital varies between 5 to 7 days with some short-term rehabilitation. The type of craniotomy performed depends on the size and location of the AVM. The option of surgery also depends on the general health of the patient. The advantage of surgical treatment is that a cure is immediate if all the AVM is removed. Disadvantages include risk of bleeding, damage to nearby brain tissue, and stroke to other areas of the brain once removed.
